患者さまの権利

1. 適切な医療を公平に受ける権利
   患者様は、個人的な背景の違いや病気の性質などにかかわらず、公平に必要な医療を受ける権利があります。
2. 選択の自由の権利
   患者様は、自らの意思で病院や医師を自由に選択・変更する権利、他の医師の意見を求める権利（セカンドオピニオン）があります。
3. 十分な説明を受ける権利
   患者様は、治療の方針や病気の内容などについて、十分な説明や情報を受ける権利があります。（インフォームドコンセント）
4. 自己決定の権利
   患者様は、十分な説明と情報提供を受けた上で、治療方法などを自らの意思で選択或いは拒否する権利があります。
5. プライバシーの保護と秘密保持に関する権利
   患者様は、診療の過程で得られた個人情報や医療上の秘密（プライバシー）が守られる権利があります。
6. 情報の開示を求める権利
   患者様は、カルテ（診療記録）の開示を求めたり、関連法規や病院の諸規則などを知る権利があります。

Patients’ right

This hospital sets “patients’ right” as here, protects this and does medical treatment aiming at the realization so that patients can have participate in medical treatment aggressively personally under an idea of “medical treatment in the patient center”.

1. The right to receive appropriate medical treatment fairly
   Patients have the right to receive necessary medical treatment fairly in spite of the difference in the personal backgrounds and the sick nature.
2. Free right of choice
   Patients have the right to choose and change the hospital and the doctor freely by the will and the right to ask doctor’s other opinions (second opinion).
3. The right to have the enough explanation
   Patients have the enough explanation and the right to receive information about a policy of treatment and the sick contents. (Informed consent)
4. Right of own decision
   After patients received enough explanation and dissemination of information, treatment method is chosen by its will or, there is a right to refuse.
5. Private protection and claim about confidentiality
   Patients have the personal information the process of the medical examination and treatment gave and the claim by which the secret on the medical treatment (privacy) is kept.
6. The right to ask elucidation of information
   Patients ask elucidation of a patient’s case record (medical examination and treatment record), and there is a right to know a related law and various rules of the hospital.
